Lynn LeFevers is a Certified Professional Coder and provides clients with documentation and coding audits, using current AHA Coding Clinic and Office of Inspector General (OIG) coding and documentation guidelines. These guidelines include: assignment of the International Classification of Diseases, Ninth Revision (ICD-9) codes to diagnoses; review of completeness of provider documentation, timeliness and legibility; assignment of Medicare Severity Diagnosis Related Groups (MS-DRGs) in acute care hospital settings and Medicare Severity Long-Term Care Diagnosis Related Groups (MS-LTC-DRGs) in Long-Term Acute Care Hospital settings; review of use of encoding products and of the sequencing of diagnoses and procedures. Ms. LeFevers has performed over 2,500 medical record and documentation audits of long-term acute care hospital patients at LTACH facilities throughout the U.S. She also provides physician and staff training on MS-DRGs, patient documentation and coding, case management and utilization review.

Ms. LeFevers has over 20 years of experience in the healthcare industry, including 14 years in clinical leadership positions.

She held several executive level positions for a post acute health care system that provided long-term acute care, rehabilitation, skilled nursing, assisted living, and outpatient services. She has served as Chief Nursing Executive, Director of Quality, Clinical Outcomes, and Accreditation, as well as providing leadership for respiratory and cardiopulmonary rehabilitation, supply/procurement, case management/social work, admitting and assessment, and infection control services.

Ms. LeFevers also served as an executive champion and clinical leader for the selection, design, project management, and implementation of an enterprise-wide clinical and financial information system. She has experience in the planning, selection, design, and implementation of automated systems for laboratory, microbiology, pharmacy, point of care testing and pharmacy robotics distribution.

She also has prepared applications for healthcare awards including JCAHO’s Codman Award, Sodexho’s Service Excellence Award, and the Ohio Award for Excellence and has given numerous presentations on healthcare quality and technology for healthcare professionals.

Ms. LeFevers received her nursing degree in 1985 from the Jewish Hospital School of Nursing, Cincinnati, Ohio; obtained a B.S. degree in 1997 from Regency College, Albany, New York; obtained Gerontological Nurse Certification from the American Nurses Association in 1995 and Certification as a Professional in Health Care Quality in 1999. She has made several presentations on clinical information systems and electronic incident reporting and is proficient in evaluation of clinical quality and risk management data and information.
